Who Needs to Complete This?

All artists from newborn up to compulsory school leaving age must complete a Child Performance Licence Form before we can apply for work licences.

Step 1 – Complete the Form

Please:

• Complete all relevant fields

• Write N/A where a section does not apply

• Do NOT date the form

• Do NOT sign where it says “Applicant” (this is signed by the production company once confirmed for work)

⚠ Your signature must be handwritten or digitally signed — do not type your name.

You may sign digitally using apps such as PDFGear, or print, sign, and upload a clear photo.

Step 2 – Upload Your Documents

Log into your Tagmin account (desktop mode recommended):

Personal → Legal & Financial → Upload/View DBS

Please upload:

• Completed Licence Form

• Copy of Birth Certificate, or passport

Please clearly label your files, for example:

• Licence Form
• Birth Certificate
• Passport

Important

• Do NOT contact your local council — Bizzy Agency handles all licence applications.

• Do NOT upload passport images, we create those for your application.

• Please upload documents yourself rather than emailing them to us. This helps us prioritise essential agency duties.
